Hospital Discharge

One of the key services we offer at NISE Nursing is managed support for service users preparing for, or following discharge from hospital. At a time of increasing pressure on NHS resources it is imperative that hospitals are efficient as possible in managing the available bed spaces open to them.

A significant part of this process is the safe movement into the community of patients who no longer require the intense care provided as an inpatient but require some clinical support within their homes.

Our team work within the current Community Care Provision, integrating seamlessly to produce a high quality outcome for patients whilst ensuring the most efficient use of hospital bed spaces.  

We are able to offer hospital managers, discharge coordinators and their patients a package of care which includes:

  • Full pre-discharge assessment
  • Ongoing care planning
  • Qualified and experienced staff to implement discharge processes
